New Student Registration


Step 1:  Complete the New Student Inquiry Form

Step 2:  Complete the Residency Verification Checklist
Residency Affidavit Forms (please complete the FIRST FORM and any other additional forms IF NECESSARY and send in (them) in with your checklist).
Required Affidavit for ALL New Students
(If Applicable) Additional Affidavit for Divorced/Separated Parent
(If Applicable) Additional Affidavit for Non-Resident Living with D29 Resident
(If Applicable) Additional Affidavit for Non-Parent Seeking to Enroll a Student

Step 3:  Bring all paperwork from the checklist to the front office for processing.  
            During COVID-19 Closure, you may mail all documents to:

**For Kindergarten Registration, you may bring the forms with you to registration.** 

Step 4:  PowerSchool Registration Form 
Once the school has reviewed and accepted your application, you will receive an email with information and a step by step guide on how to create a Parent PowerSchool account and how to complete the online registration form.  Once this form has been completed, your child will be all set to attend school. The start date will be set by the administrator.  

Step 5:  Print out all Health Office Forms
You can find them on the registration form or from our website (under Schools>Middlefork or Sunset Ridge>Health Services.  All allergy or asthma plans must be completed and turned into the front office.

Homeless Children & Families
Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or for information and assistance with registration.
