DISTRICT 29 KINDERGARTEN REGISTRATION


We are happy to have you join us in District 29!  Please click below to sign up to attend our Kindergarten Registration event.  In addition, please see the requirements below and bring the appropriate paperwork to the event. 

***Prior to attending the Kindergarten Registration Event you must complete the Online Registration Form.***



The Kindergarten Registration Event is on Wednesday, January 24th from 3:30pm-6:30pm @ Middlefork School. Click Here or scan QR code below to make an appointment for the 24-25 Kindergarten Registration event.

The following steps must be completed in order for your child to be enrolled for the 24-25 school year.

Kindergarten Registration

  • Step One: Student Online Registration Form

    • Please complete the online registration form here. Each child must have their own registration completed.   
    • Sign in if you already have an account with our District or click "forgot password"
    • Create an account if this is your first child attending Sunset Ridge School District 29
    • Complete all "required" fields
    • Submit the Registration Form

Included in the Registration Form: 

  • Student Information 

  • Agreements

  • Health Forms - some of these need to be printed out and turned into the front office (i.e. allergy medical plans, asthma medical plans, etc.)

  • If you did not print them out from the registration form, you can access all forms on the website under the Schools menu, Middlefork or Sunset Ridge, Health Services.

 

  • Step two: Residency Verification 

  1. Please complete this checklist for residency verification.  Please print out the checklist and bring the completed checklist and all paperwork to the front office.

  2. Complete the FIRST affidavit form and any additional forms IF NECESSARY and bring it with you.

            (If Applicable) Additional Affidavit for Divorced/Separated Parent
            (If Applicable) Additional Affidavit for Non-Resident Living with D29 Resident
            (If Applicable) Additional Affidavit for Non-Parent Seeking to Enroll a Student
  

  • Step 3: New Student History Form

Please complete the New Student History Form so we can get to know  your child.

Homeless Children & Families
Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or [email protected] for information and assistance with registration.
