Kindergarten Registration Paperwork

  • Residency Verification 

  1. Please complete this checklist for residency verification.  Please print out the checklist and bring the completed checklist and all paperwork to the front office.

  2. Complete the FIRST affidavit form and any additional forms IF NECESSARY and bring it with you.

            Required Affidavit for ALL New Students
            (If Applicable) Additional Affidavit for Divorced/Separated Parent
            (If Applicable) Additional Affidavit for Non-Resident Living with D29 Resident
            (If Applicable) Additional Affidavit for Non-Parent Seeking to Enroll a Student
 

  • Student Online Registration

  1. A link will be sent to you - Please complete the link that was sent to you from one of our Administrative Assistants.  Each child must have their own registration completed.   

Included in the Registration Form: 

  • Student Information

  • Agreements

  • Health Forms - some of these need to be printed out and turned into the front office (i.e. allergy medical plans, asthma medical plans, etc.)

  • If you did not print them out from the registration form, you can access all forms on the website under the Schools menu, Middlefork or Sunset Ridge, Health Services.

  • New Student History Form

Please complete the New Student History Form so we can get to know  your child.

Homeless Children & Families
Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or [email protected] for information and assistance with registration.
