The State of Illinois allows for two kinds of exemptions. See below for information regarding each kind of exemption:

1) MEDICAL EXEMPTION: If your student has a medical contraindication for any of the required Illinois State Vaccines, please. have your health care provider write a letter stating the following:
- a list of vaccines that are medically contraindicated
- the reason why they are medically contraindicated
Should the medical condition of the child later indicate that immunization is no longer contraindicated to the health of the child, the immunization requirements will then have to be met. Please submit this letter along with the physical before October 15 to avoid school exclusion.
