Student Registration

26-27 Kindergarten Registration

The following steps must be completed in order for your child to be enrolled for the 26-27 school year.

Complete the steps 1, 2 and 3 below.

Bring Residency Verficiation Paperwork to the Middlefork School Kindergarten Registration Night.

 

    • Step One: Student Online Registration Form
    1. Please complete the online registration form hereEach child must have their own registration completed.
    • Sign in if you already have an account with our District or click "forgot password"
    • Create an account if this is your first child attending Sunset Ridge School District 29
    • Complete all "required" fields
    • Submit the Registration Form

     

    Included in the Registration Form: 

    • Student Information 
    • Agreements
    • Health Forms - some of these need to be printed out and turned into the front office (i.e. allergy medical plans, asthma medical plans, etc.)
    • If you did not print them out from the registration form, you can access all forms on the website under the Schools menu, Middlefork or Sunset Ridge, Health Services.
    • Step two: Residency Verification 

    1. Please complete this checklist for residency verification.  Please print out the checklist and bring the completed checklist and all paperwork to the front office.
    2. Complete the FIRST affidavit form and any additional forms IF NECESSARY and bring it with you to Middlefork on Registration Night or later to the District Office. Click here for a Registration Night slot.

    • Online registration form questions contact Ken Neumann at (neumannk@srd29.org)
  • Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or stangee@srd29.org for information and assistance with registration.
