25-26 Kindergarten Registration
DISTRICT 29 KINDERGARTEN REGISTRATION
The following steps must be completed in order for your child to be enrolled for the 25-26 school year.
Kindergarten Registration

- Step One: Student Online Registration Form
- Please complete the online registration form here. Each child must have their own registration completed.
- Sign in if you already have an account with our District or click "forgot password"
- Create an account if this is your first child attending Sunset Ridge School District 29
- Complete all "required" fields
- Submit the Registration Form
Included in the Registration Form:
- Student Information
- Agreements
- Health Forms - some of these need to be printed out and turned into the front office (i.e. allergy medical plans, asthma medical plans, etc.)
- If you did not print them out from the registration form, you can access all forms on the website under the Schools menu, Middlefork or Sunset Ridge, Health Services.

Step two: Residency Verification
- Please complete this checklist for residency verification. Please print out the checklist and bring the completed checklist and all paperwork to the front office.
- Complete the FIRST affidavit form and any additional forms IF NECESSARY and bring it with you.

Step 3: New Student History Form

Please complete the New Student History Form so we can get to know your child.

Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or stangee@srd29.org for information and assistance with registration.

New Student Registration
STEPS TO REGISTER A NEW STUDENT ENTERING DISTRICT 29
(HAS NEVER ATTENDED BEFORE & Entering Grades 1st – 8th Grade)
25-26 School Year Only

Step 1: Complete the New Student Inquiry Form.
Step 2: Complete the Residency Verification Checklist.
Residency Affidavit Forms (please complete the FIRST FORM and any other additional forms IF NECESSARY and send in (them) in with your checklist).- Required Affidavit for ALL New Students
- (If Applicable) Additional Affidavit for Divorced/Separated Parent
- (If Applicable) Additional Affidavit for Non-Resident Living with D29 Resident
- (If Applicable) Additional Affidavit for Non-Parent Seeking to Enroll a Student
Step 3: Bring all paperwork from the checklist to the front office for processing.
Step 4: PowerSchool Registration Form
Once the school has reviewed and accepted your application, you will receive an email with information and a step by step guide on how to create a Parent PowerSchool account and how to complete the online registration form. Once this form has been completed, your child will be all set to attend school. The start date will be set by the administrator.
Step 5: Health Forms
Please print out all necessary Health Forms for your child and bring them to the front office.
