Student Registration
26-27 New Student Registration
The following steps must be completed in order for your child to be enrolled for the 26-27 school year.
Complete the steps 1 through 4 below.

- Step One: Complete New Student Inquiry Form
- Please complete the New Student Inquiry Form.

Step two: Residency Verification
- Please complete this checklist for residency verification. Please print out the checklist and bring the completed checklist and all paperwork to the front office.
- Complete the FIRST affidavit form and any additional forms IF NECESSARY and bring it to the District office.

Step 3: New Student History Form(MFS Only)

Please complete the New Student History Form if your student will be attending Middlefork School so we can get to know your child.

New Parent Guide
Please visit our New Parent Guide under the Parent Menu on our website.

You will receive an email once residency has been verified that includes a link to the registration form. Each child must have their own registration completed.
Included in the Registration Form:
- Student Information
- Agreements
- Health Forms - some of these need to be printed out and turned into the front office (i.e. allergy medical plans, asthma medical plans, etc.)
- If you did not print them out from the registration form, you can access all forms on the website under the Schools menu, Middlefork or Sunset Ridge, Health Services.
TIPS:
- Sign in if you already have an account with our District or click "forgot password"
- Create an account if this is your first child attending Sunset Ridge School District 29
- Complete all "required" fields
- Submit the Registration Form

Per District 29 Board Policy 713, any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce records normally required to establish residency. Homeless individuals should contact the District 29 McKinney-Vento Coordinator (Dr. Edward J. Stange) at 847.881.9546 or stangee@srd29.org for information and assistance with registration.
